The effectiveness and feasibility of fluid resuscitation directed by microcirculation monitoring in patients with septic shock: a randomized controlled trial
نویسندگان
چکیده
Background: This study sought to examine fluid resuscitation in septic shock patients by monitoring their sublabial point of care microcirculation score (POEM) scores (a 3.5 cut-off value was used as the end recovery). It also explore effectiveness and safety using POEM shock.

Targeting the microcirculation in resuscitation of patients with septic shock: fluid is superior to norepinephrine
Methods This was a single centre prospective study. Patients in septic shock for less than 24 hours despite fluid resuscitation were enrolled. Passive leg raising test was performed to predict fluid responsiveness. Responders were randomizedly divided into two groups, i.e. Resp-FL group and Resp-NE group to maintain usual MAP. In Resp-FL group, patients were treated by fluid until with no respo...
متن کاملShould microcirculation monitoring be used to guide fluid resuscitation in severe sepsis and septic shock?
Tissue hypoperfusion and subsequent limited oxygen transport are critical features conducting to organ failure during shock states. Therefore, early identification of tissue hypoperfusion and adequate resuscitation are key for improving the probability of survival after septic shock.(1,2) However, how to identify organ perfusion abnormalities at the bedside and select the type and amount of flu...
متن کاملThe effect of physiologic dose of intravenous hydrocortisone in patients with refractory septic shock: a randomized control trial
Objective: Septic shock is a response to infection and tissue hypoperfusion which does not respond to fluid therapy and eventually leads to organ dysfunction. Aggressive treatment of a broad-spectrum antimicrobial and supportive measures are the cornerstones of successful treatment. In addition to the main treatment, there are adjunctive therapies. Steroids are one of the treat...
متن کاملLevosimendan for resuscitating the microcirculation in patients with septic shock: a randomized controlled study
INTRODUCTION The purpose of the present study was to investigate microcirculatory blood flow in patients with septic shock treated with levosimendan as compared to an active comparator drug (i.e. dobutamine). The primary end point was a difference of ≥ 20% in the microvascular flow index of small vessels (MFIs) among groups. METHODS The study was designed as a prospective, randomized, double-...
متن کاملTrial of early, goal-directed resuscitation for septic shock.
BACKGROUND Early, goal-directed therapy (EGDT) is recommended in international guidelines for the resuscitation of patients presenting with early septic shock. However, adoption has been limited, and uncertainty about its effectiveness remains. METHODS We conducted a pragmatic randomized trial with an integrated cost-effectiveness analysis in 56 hospitals in England. Patients were randomly as...
